Position Summary
As a Care Coordinator at Toriven™, you are the first point of contact for families and individuals seeking care. Your role is to understand each inquiry, guide patients to the most appropriate service, and aim to complete the booking process during the initial interaction where appropriate, while ensuring accurate service matching.
This is a high-responsibility, high-impact role within a multidisciplinary, private-pay environment. You will work across psychology, speech-language pathology, therapy, and learning support services, ensuring each patient is matched appropriately and supported through the intake process.
This is a non-clinical role focused on care coordination and intake. The Care Coordinator does not provide clinical advice, diagnosis, or treatment recommendations. All clinical decisions are made by regulated healthcare professionals, and cases are escalated as appropriate.
This is a virtual role. Occasional in-person team meetings or events may be required (e.g., annual or semi-annual gatherings).
What You Will Do
Intake & Patient Guidance
- Respond to new inquiries (phone, email, web) and conduct structured intake conversations.
- Understand presenting concerns and guide patients to appropriate services using defined intake pathways.
- Clearly explain available services in simple, non-clinical language.
- Match patients to the appropriate clinician based on service type, age group, and presenting need.
- Aim to complete booking during the same interaction where appropriate.
- Identify when a case requires escalation to a clinician and coordinate accordingly.
- Provide guidance using structured intake pathways without making clinical assessments or diagnoses.
Booking & Intake Completion
- Book appointments accurately within the EMR system (e.g., Jane App or equivalent).
- Ensure intake forms, consent, and required documentation are completed prior to the first visit.
- Provide clear next steps to patients, including appointment preparation and timelines.
- Support patients with questions related to services, scheduling, and process.
Patient Experience & Follow-Up
- Maintain a calm, professional, and empathetic tone in all patient interactions.
- Follow up on incomplete bookings or outstanding intake requirements.
- Identify and flag complex or high-risk cases for escalation.
- Support a seamless, high-quality intake experience across all service lines.
What You Bring
- Experience in multidisciplinary clinic coordination, intake, or care navigation (psychology, mental health, therapy, allied health — private practice preferred).
- Strong ability to manage intake conversations and guide patients across multiple services.
- Excellent communication skills — able to explain services clearly without clinical jargon.
- Experience working in a fast-paced, private-pay or outpatient clinic environment.
- High attention to detail in booking, documentation, and follow-up.
- Ability to work with structured processes and decision frameworks.
- Comfortable using EMR systems (Jane App, Juvonno, or similar platforms).
Nice to Have (Not Required)
- Background as RPN, RP, RSW, or other regulated healthcare professional.
- Experience supporting psychology assessments, SLP services, or multidisciplinary programs.
